Terms of Use

Promotions:

​

i. Comprehensive check up, clean & fluoride

     a. This can only be used twice a year.

     b. X-rays may be taken, only if necessary, however not included in the promotion.

     c. Gap free means as long as your private health insurance pays up to $202 for a check up, clean & fluoride,

         we will waive the gap for you. If your private health insurance covers more than $202, we can not pay you

         back the gap above $202. If you have any questions, please call our friendly reception.

     d. Your private health insurance must cover at least $202. If your rebate is less than $202, you would need to

         pay the difference up to $202.

     e. This offer can not be used with any other offer nor can it be substituted for cash.

     f. The patient needs to mention this promotion when booking an appointment to redeem the offer.

ii. Teeth whitening

     a. This can only be used once a year.

     b. Before commencement of procedure, a patient must have a full mouth check up and X-ray to ensure that 

         there is no tooth decay.

     c. Teeth whitening does not whiten existing fillings and crowns, but rather only the structure of the tooth.

         White fillings, porcelain or composite restorations, crowns and veneers may not whiten at all or evenly

         with your natural teeth during this procedure.

     d. During the procedure, you may experience tooth sensitivity, so if your teeth are normally sensitive, please

         inform us before treatment.

     e. You may experience some inflammation of the gums, lips and cheek margins. This is generally a result

         of the whitening gel coming into contact with these tissues. Therefore, protective materials are placed

         in the mouth to prevent this.

     f. Chair side teeth whitening is not recommended for pregnant or lactating women, children under 14 years

        of age or any known peroxide allergies.

     g. The patient needs to mention this promotion when booking an appointment to redeem the offer.

iii. Dental crystals

     a. As with all procedures, there are certain potential problems associated with placing dental crystals. Etching

         of the tooth enamel which may lead to a slight sensitivity for the next couple of days.

     b. The practice of cosmetic dentistry is not an exact science and you should acknowledge that no guarantees

         can be made to you by your dentist concerning the success of the treatment.

     c. You may require ongoing maintenance such as re-cementing the dental crystal back on the tooth.

     d. The longevity is directly related to what you eat, drink, home care habits and maintaining regular bi-annual

         recall appointments.

     e. The patient needs to mention this promotion when booking an appointment to redeem the offer.

iv. Sports mouth guards

     a. Patients in need of a mouth guard must be cleared satisfactory for healthy permanent teeth.

     b. Patients should have a full set of permanent teeth.

     c. The patient needs to mention this promotion when booking an appointment to redeem the offer.

v. Porcelain veneers

     a. Patients are advised that prior to proceeding with this option, a comprehensive check up in addition to an  

         X-ray are needed for thorough assessment of the teeth.

     b. The patient needs to mention this promotion when booking an appointment to redeem the offer.

vi. Concession card holders

     a. The patient must present a valid concession card at the time of their appointment.

     b. The patient needs to mention this promotion when booking an appointment to redeem this offer.

     c. Concession card holders are eligible for 20% their treatment if it is above $1000.

​

vii. Private health insurance card holders

     a. The patient must present a valid health insurance card at the time of their appointment.

     b. The patient needs to mention this promotion when booking an appointment to redeem the offer.

viii. CDBS

     a. Children must be eligible for the Child Dental Benefits Scheme. Our friendly staff is able to help you find

         out your child's eligibility.

     b. A valid medicare card is required. Parents/guardians should sign a bulk bill form beforehand.

     c. After the treatment plan is finalized, if the total is below $1000, we can not pay you the remainder in cash

         and if the total is above $1000, the parent/guardian is required to sign a non-bulk bill consent form so the

         treatment can be finalised.

     d. The patient needs to mention this promotion when booking an appointment to redeem the offer.

Clinic policies:

ix. Cancellation policy

     a. Any cancellation within 24 hours of the appointment time will incur a $50 fee.

     b. We understand that patients require cancellation of their appointments from time to time, however, the

         clinic allows a maximum of three consecutive cancellations. Above this, patients can only be booked for

         emergency appointments.

 

x. Zero tolerance to workplace disrespect and aggression

a.     Our clinic holds zero tolerance for disrespectful or aggressive behavior towards staff and patients.

​

xi. Outstanding balance

a.     An outstanding balance not paid within 30 days may result in the cancellation of your appointment until you

        are able to take care of the remainder of the account.
